Problem with HP vp17 (blacking out)

I have a6125in upgraded  with nvidia 9500GT.I have  HP vp17 Monitor ,which is giving me problem . It is blacking out repeatedly for a short time then goes back to normal, but again after some gives problem to me. Initially it was connected to 9500GT via DVI cable, then i changed cable to VGA but no improvement and then i switched to onboard GMA but still no improvement. Please someone help me.Is my monitor gone bad?
