this is a manufacturign error and it seems to appear after the warranty has worn off... coincidence?

Had the same problem. Exhausted all possible avenues, including 3rd pty applications installed recently, updating drivers, "clean boot" , and everything else I read on the internet. Problem is my case is the hardware, ie, the hp monitor for my pc. When I pressed the menu and auto buttons simultaneously, the volume bar would disappear, but when I release the buttons, it would reappear. Got a can of compressed air and shot each of 5 buttons to see if there was dust particles behind each button that I might dislodge. Presto!! No more volume bar in the middle of my screen. Hope this works for many of you.

Like others, unplugging the circuit board fixed it.
You remove the front black plastic rectangle that frames the screen. I inserted a flat head under it and twisted gently all around. It didn't take much.
Once completely off, I held the jack down with one screwdriver and used a second one to pull up on the wires.
It's on the left side end. The wires were red and black.
The black trim snaps right back on.
I suppose the buttons could be removed and cleaned to get the board working again but it would take a lot more work to actually remove the board vs just unplugging it.
Canned air might work as others have stated but it's $6+ at Office Max.
