Officejet 4580 skips the 1st & 3rd lines of ink

Officejet 4580 skips the 1st & 3rd lines of ink on printed pages, i.e. whatever is at the top of the page, has a blank space that runs through it. (toner cartridges are new. Using Mac OSX).
