At start up I get an error that my hard drive is failing, I click continue and everything seems to run just fine. After a minute or so and every few minutes after that I get an alert saying "disk on port 0: at risk Reset Disk to normal". I do this and the error goes away only to return in a few minutes. What can I do?

Replace the hard drive. When you reset it you just clear the prior SMART errors but as soon as another one is generated you get the message again and will continue to do so until the drive fails.
