What I do, is I take the computer outside...then I blow out the entire case with a can of compressed air. Make sure that when you blow out the fans that you do it gently - you don't want to damage the bearings in the fans...that would make the fans noisy. Also, some of the older fans may have dirt compounded on them and it can't be blown off easily, this can create tracks in the dirt which cause the fans to make a lot of noise when used. The fan attached to the silver fines (heatsink) is the CPU fan, you can blow it out as well.
Alternatively, you can choose to just replace the fans with better fans...they're usually pretty cheap and easy to replace.

Basically, the air should be blowing outwards so that the air is pulled out of the unit. The top of the fan should be facing outward, while the underside should be facing the heatsink or the inside of the case. The image below shows the top side of a 120mm fan (the side that face up or outwards).
