Sound Coming From Both Headphone and Speaker (Elitebook 8460p)

I have windows 7 64-bit and when I plug headphones into the jack in the front right, the sound comes out of both the speaker and headphones. Here's what I've tried to fix the problem:

 

1) BIOS update to P20

2) Audio driver update to latest on HP site

3) Make headphones not the communication default

 

I'm running out of options here, and it seems like nothing will solve this. Thanks

I've got the same problem as well. It's very frustrating. Does anyone have an answer?
