Hi there. Whenever click "open" or "save as" in microsoft word, powerpoint, photoshop, itunes... my folders are missing icons. Individual files still have theie icons but the folders do not. Attached is a screenshot. I have booted in safe mode, performed "sfc /scannow", and ran Norton antivirus. None of these programs found any irregularities so I am sure that this is not a third-party attack on my computer. Any ideas?

This is in your folder options or list options. You can check the "View by:" options in the window or goto your Control panel then folder options.

So I decided to right click on a folder, choose properties, go to customize, then "change icon". What is interesting is that the selected icon is the "unrecognized icon". I tried changing it to another icon in that list but it would keep defaulting to the "unrecognized icon". Is there something wrong with shell32.dll or something of that like?

Sounds like some windows files have become currupt. Try useing the repair option using the Windows disk at startup.
