Black printer output is streaky, smudged, fuzzy

I have an Officejet Pro L7780 printer.  When printing, the black output is streaky, looks smudged, fuzzy, and sometimes is missing.  Color output is always OK.  Have the latest drivers for Windows 7 installed.   Self-test diagnostic page says printhead status is good.  Windows printer test page shows color OK, black output bad.  Changed cartridge, no results.  Cleaned print heads 3 times, no results.  Tried aligning printer heads, a fault is detected.

 

Any ideas?

Thanks, I have previously reviewed this document.  It does not address the problem I am experiencing.  Color prints OK.  Black is smudged, fuzzy or missing.  Shows up this way when you print out the status report.  Not a program problem.  Not a paper problem.  Using HP cartridges.
