Photosmart C4500 series Printing Paper Jam Problem

My Photosmart C4500 series printer randomly begun paper jamming and I can't find a solution to this problem.

At first there was a cartridge jam error, fixed that, and now there are a sequence of paper jams. The problem is the paper feeds wrong and sandwiches in-between the printer as it feeds through to print. It is not necessary a "paper jam" problem, seeing every time I pull the paper out correctly after it jams it just happens all over again once I put new paper in the printer and queue the print job.

Is there any way to fix this problem? I insert the paper to the printer fine and flat, but every time it feeds through the printer it doesn't go through straight, sandwiches in half mid-way through the printer, and jams up. Help?
