how to stop printing banner page on photosmart D110a

My printer Photosmart prints banner page with every printing and there is no options to stop printing banner page which prints on the page file name, uthor name , creation day and number of pages. Please help me to stop printing banner page. Printer is HP Photosmart D110a Model number CN731 installed on Windows 7

Has your printer always done this or did it just start doing it ?

 

Does it print that page when you print a document, photo, or both ?
