What is the form factor for my PC? Would this memory card work and can I keep my old HDD as a 2nd hard drive? Thanks!!

Yes, the 2.5" SSD will work in your computer.  You will need to use an adapter for 2.5" to 3.5" drive bay: https://www.walmart.com/ip/Black-2-5-SSD-to-3-5-Bay-Hard-Drive-HDD-Mounting-Dock-Tray-Bracket-Adapte...

Yes, both items should be fine.  The SATA power cable will be connected to your power supply unit (PSU).  Good luck!
