Heat Sink Fan

My heat sink cooling fan speeds up intermittently and sometimes when I click on something or coming out of sleep.
Everything was fine until a few months ago
I keep the inside clean periodically.
Is it the fan or heat sink?? or something else?

When was the last time you cleaned the fan with compressed air?
With a finger in the blades to keep it from spinning (to prevent damage) use compressed air to blow out the dust. You can use pipe cleaners to get into the area between the heat sink fins.
Do not remove the heatsink, as that is not required. Actually, the fan is held by screws. Look through the top screws holes into the heatsink.
