Need to get RMA for a failing EX900 SSD

HP Recommended
HP SSD EX900 M.2 120GB

I have an SSD that is having a lot of critical medium errors and controller resets occurring. I tried verifying the warranty to start the RMA process, but get "HP was unable to find a match based on the information provided". I purchased the SSD new last year and it has a 3yr warranty so I'm sure it is covered.
