Download and used MediaCreationTool1909 from Microsoft website.

Installation goes but at about 40% reported completion on screen,

a message "Windows 10 installation has failed" appears.

No other fail codes appear to indicate what went wrong.

 

How do you to debug and resolve this issue?

Dear Alvin,

 That sounds like critical (code) errors on your HDD. Find a windows boot USB with scandisk on it.

 

 From the USB boot. Blank screen - cursor flashing - type  Scandisk C: /f/r    then hit the  'return' or  'enter' key ... and let it run.

 

 Can take quite a few hours, depending on the size/capacity of the hard drive. It does not destroy your data/files - simply fixes code errors and faults that are on the HDD ... and is successful 98% of the time. Sometimes you can get a broken string of machine code that is too long for scandisk to handle (the code begins to interfere with scandisk), and the disk cannot be recovered with normal software or hardware (need special forensic data recovery programs) - but that is rare.

 

 Once it finished you will see a "completed" log, showing what it did/fixed.

 Tell it you're really impressed ... then turn-off the computer, remove the USB, turn it on and after it had booted, start your installation process again.

 

 It should complete with no further dramas.

 

 If you get the same error message at 40%, it means your installation program/files are damaged or compromised. Get a new copy and start again.

Windows installation failure is almost always due to a failing drive.

If your PC is new enough to support UEFI, you can use these steps to test the hard drive: http://support.hp.com/us-en/document/c00439024

If not, to check the drive you have to follow these steps:
1) Press Esc key repeatedly, several times a second, while rebooting the laptop. Do NOT hold the key down, just press it over and over.
2) Eventually, you will see an HP Startup Menu
3) Press the Function key for testing the hard drive (usually F2) and let it run.

Sorry for the long delay in responding. Thank you for trying to help. However this is not a laptop. It is a desktop PC. After researching the online community and trying what seems like dozens of suggestions, it took me quite a while before i was able to overcome one issue after another issue; by debugging one error after another error in the upgrade error logs; almost endless but finally able to successfully upgrade from w7 to w10 a few days ago. Looking back it was not just one thing but many issues. Anyway thank you. Now to move on to some other minor w10 error with hex 0x80070005 it seems related to folder or registry access denied issues. Still researching.

Your idea of running a disk scan for disk errors may have helped to move upgrading w7 to w10 along but i did some other suggested fixes as well so can't say what actually is the issue. Thank you for the suggestion.
