Product: HP ENVY 5000 all in one series

My printer control panel is only showing horizontal lines . I don’t know if this is because l downloaded the printer app to another device . Unfortunately I don’t know if the screen was like this before I started or not . My device is an apple iPad and   I haven’t had any problems till now . I downloaded the printer app to a Samsung tablet and have now removed it . The printer is printing from my iPad but the control screen just won’t reset . I have tried turning the printer off and on again but that hasn’t worked . I am now thinking could I get it back to factory settings but I don’t know how to do that . Any help appreciated.
