Why am I charged at the higher rate plan one more time before the change to the plan takes effect in the next billing cycle? Wasn't I charged in advance when I signed up or am I mistaken and I was and am charged in arrears?

The Instant Ink plan bills in arrears, after the usage, so that any overage costs can be included. This is described in section 7 of the terms of service here.
