Black ink cartridge has completely jammed, with 1/2 ink supply remaining.

I am a member of Instant Ink.  My black ink cartridge has completely jammed and will not print anything, despite having 1/2 ink supply remaining.  I have tried all troubleshooting steps, included cleaning cartridge 3 times, but everything has failed.  How do I get a replacement cartridge sent, since Instant Ink only responds to the amount of ink remaining, not the condition of the cartridge.  Thank you.
