Welp, I'm on a mac.

Instead I searched through the forums and found info that the default driver (v2.0) on the macs may be buggy, so I deleted all my printers, reset my printing system and loading the driver (v4.4) from the HP website.  Since the problem appears intermittently, I won't know if it's fixed for a few days.  

Seems like this should be resolved between Apple and HP, not just some random info buried in a forum on here??

I see that you have issues with the first page printed since you've already performed most of the steps, I'd recommend you follow the steps below-

 

Update the printer firmware

Download available firmware updates from the HP website

With the printer on and connected to the computer, go to HP Customer Support - Software and Driver Downloads, and then identify your printer. Look for and install any available firmware updates. Refer to the document HP Printers - Updating or Upgrading Printer Firmware

 

What happens when you make photocopies on the printer? Is the first copy blurry as well?
