Product: HP OfficeJet Pro 6978 All-in-One Printer

My all in one printer has started to jam up when attempting to scan using the autofeed .  It actually causes a rip in the paper in the direction of the feed in the middle of the page.  I can't find anything causing it to occur.  Looking to fix this problem asap.
