I had this on going problem for about a year, No one picked it up till I was talking to a friend about this.
something as simple as move the unit closer to the computer and use a shorter cable.
I bought a longer phone cord and a shorter USB cable.
Since it won't copy also, sounds like a problem with the unit. Contact HP Support have them check it out for you.

Hi Cliff,
Today morning i came across the same issue, My HP OJ 4500 g prrinter's scanner isn't functioning.
The carriage beneath the glass isn't moving at all. I think it gets jammed near the starting piont (right side corner). whenever we give a print, fax or copying job.
Please assisi me on this..

My Officejet 4500 had a paper jam during copying through the paper feeder. The paper was removed gently but now the orange light flashes and the screen says "scanner failure, unable to copy, scan or fax.
I have followed all the HP solutions but none work. I just bought this machine 3 months ago !.
Any suggestions for resolution ?

For those who are able to scan or copy but receive blank pages, you are out of ink.
For those who are having trouble getting a scanning failure, unplug your cord from powerbar and plug into a regular plugin and check all your connections are right. The scanner is not getting enough power or something has given it diminished power. Turn back on and wait for everything to warm up. Then try it, if everything works you can probably plug back into your power bar.
It will print from the computer, because the computer will give it that little extra power.
Hope this helps, it is what worked for me after searching many answers.

HELP I have an HP J4540 All In One that will not scan or copy. Gives the "Scanner Failure Message.

Try downloading and running the print and scan doctor located here:

It can fix a lot on its own and if not give a better idea of what is going on.

I fixed it! I fixed it! I've had the same 'no scanning, no copying' on my HP Officejet 4500 for a long time. I'm not sure what I did to fix it. (sorry) I took the printer apart. Removed just 6 screws and was able to access the 'bulb area'. I thought the problem was that the printer did not sense enough heat from the bulb, preventing copying & scanning. I removed the ribbon connector from the under the control panel, you know, the panel with the numbers, etc. I felt that the ribbon was loose. Anyway, I pulled the ribbon from the console and then re-inserted it. Without putting the printer back together, I scanned and printed a document. It worked. I put the printer back together and now I can copy & print. (I don't fax) Good luck.
