Officejet Stopped Sending Faxes

My Officejet stopped sending faxes, for no apparent reason. When I print to the fax-printer, everthing seems to proceed as normal (selecting fax number, adding cover page). After I hit send, nothing happens. I've run the Fax Test Report, and I get all "PASS". When I go to the HP Printer Assistant > Manage Fax Settings > Fax History, it shows the faxes I sucessfuly sent last month, but nothing about any of the recent (unsuccessful) faxes I've attempted in the past few days. I tired restting the printer (unplugging and replugging), but that doesn't help. Any suggestions?

Update: I find I can send a fax if I use "Send a Fax" from the HP Printer Assistant (and it then appears in the Fax History). This is inconvenient, as to send a PDF file, I have to print it first and the scan it.

In printers, I noticed a "phantom" printer: HPC1A025 (HP Officejet Pro 8600). I removed it and was then able to fax again! (I mean that I can now use the "HP OJ Pro 8600+ Fax" printer to "print" my PDF documents again.) Problem is gone!
