Scanned page ends up black and full of vertical lines

Microsoft Windows 8.1 (32-bit)

When i scan a document with HP Smart app the document, both printed and soft copy, shows a black page with vertical lines only. I have up dated all drivers, software, firmware and I tried to use HP Print and Scan Doctor and only ends up saying scanner not detected. I have tried the virtual agent help of hp and to no avail. Does anyone know what else I should do?
