I have an Officejet 6500, and recently had to replace the printhead.  I purchased a new printhead, and installed it when it came in.  It worked fine for about 2 months, then periodically the printer would randomly lose track of one of the ink colors ... sometimes blue, sometimes yellow, etc.  I contacted HP and after a multi-transfer, multi-"repeat the story" episode, they said they would replace the printhead since it was within the 90 part warranty period.  I have been waiting and waiting, and finally clicked on the tracking link in the email I received when they agreed to ship the replacement:  to my surprise, there was some off-hand comment in the tracking page that said "Waiting on customer input".  I received NO email or phone call about the "input" they are waiting on, and there is no contact info on the tracking message to point me to check with someone in HP.  This is really frustrating, but sadly is typical of the extremely poor customer service I've received from HP lately.  Does anyone know who I should be talking to about this or how to contact them?
