Hello guys. A couple of weeks ago, my PC started resetting when I was playing Skyrim after having installed some mods for the game. I was receiving the BSOD with a new error code each time. It would always reset before windows could load. I tried a number of times to use the HP System Diagnostics to test my memory and hard drives, but the hard drive test would always freeze on 99% with 1 second remaining. I then decided to use the 5 system recovery disks. After the fifth disk had finished installing, the PC would reset and then a red screen would appear saying:

 

945 ERROR An error has occurred during the 1st offline phase!

Failure spawning C:\ERROR_FAILED_TO_APPLY_BASEIMAGE.EXE

CreateProcess(C:\ERROR_FAILED_TO_APPLY_BASEIMAGE.EXE) returned 2, The system cannot find the file specified

Open command window and collect c:\system.sav.7z for issue reference.

 

I’m completely at a loss as to what to do. The PC was fine until I started playing a modded Skyrim. I tried calling HP in the UK but it’s a public holiday.

Let me take a moment to thank you for posting on the HP Support Forums. I came across your post and would like to help. 

 

I understand that you have an HP ENVY Phoenix Desktop - 860-078na PC and initially your PC PC started resetting while playing Skyrim after installing some mods for the game. The BSOD will come up each time with a new code. It would always reset before windows could load. I read that HDD test would freeze at 99% and later you performed aa system recovery using the recovery disks and it failed. 

 

I appreciate the steps you have performed and have certainly done a great job. Let me know if you have tried running a test on the Processor and System Board also:

Secondly, please ensure that while you perform a system recovery, all external devices need to disconnect. Only the power supply cable, keyboard, mouse, and monitor need to remain connected. 

 

I'm not sure fi the recovery partition on the hard drive is still intact on your PC. However, we can try to use it and check if it works. Please follow the steps mentioned under"Recovery when Windows 10 does not start correctly."

Thanks for the reply. I tried pressing F11 to start sytstem recovery, but I get instead is a blue screen with the hp logo and a box with the message "Please wait while HP software installation is initialized. Then I sometimes get the red screen with the 945 error message as I posted above. Yet sometimes I get a Blue Screen of Death with the error: BAD_POOL_HEADER and System_Service-Exception. Therefore I cannot use system recovery after pressing F11

 

I have also pushed ESC for the start-up menu and then pressed F11, and I still get the same problem.

 

Also,  I have pressed F2 for System Diagnostics. I have the BIOS version 1.16.0.0. There are only 2 tests, a memory test and a hard drive test. I tested both hard drives again and this time it completed instead of stalling on 99%. It said there were no errors.

 

I really don't have a clue what to do. I suppose I must  phone HP after New Year and maybe send in my PC for repair as I still have 3 months of warranty left.

You're welcome. I appreciate the time taken to troubleshoot. As I understand you have tried the steps suggested and still having the same issue. Don't worry as I have a few other steps to help you get through this concern,

 

Just want to check, Have you tried running the quick test or the extensive test on the memory and the Hard drive? If you've run the extensive test on the Memory and the Hard drive and as the test has passed successfully without any errors. I would suggest the next option would be to perform a system recovery. Since you are not able to access the recovery partition it could be due to system file is missing or the partition is corrupted. 

 

The only other way to install Windows would be to order the recovery media for your PC and using the recovery media you can install Windows. You can check if the recovery media is available online on www.hp.com/drivers by entering the exact model no. of your laptop. To find the exact model no. click here: http://hp.care/2bygf7l

 

Otherwise, you could also reach HP phone support to order the recovery media. Please fill in the product details to get the tech support number.

Step 3. Scroll down to "Still need help? Complete the form to select your contact options"
