Microsoft Windows 10 (64-bit)

Using Windows 10 Pro 64bit.

This machine has a front bezel accessory assembly containing two audio input/output jacks and 4 USB ports.

The 4 USB ports are cabled to a PCEAVOCCY3LBZU Rev. OC (Carver Rev A) mainboard using 2 separate cables: 1 yellow  connected to the "Front USB 1" connector, and 1 green connected to the "Front USB 2" connector. Both of these connectors are pinned differently. It is not possible to switch the cables between the mainboard port connectors.

The top 2 USB ports are cabled to the "Front USB 2" connector (green) and are failing to recognize devices. It is with these I need some help.

The bottom 2 USB ports are working normally.

When connecting any USB device to either of the top 2 USB ports, the device fails to be recognized and generates an error message in Windows, e.g., "USB device not recognized..."

Furthermore, in Device Manager/Universal Serial Bus controllers, 2 devices are instantiated with yellow exclamation points:

1) Unknown USB Device (Device Descriptor Request Failed)

2) Unknown USB Device (Port Reset Failed)

In the "Events" tab for device 1) the following is logged:

Device USB\VID_0000&PID_0002\6&163e55af&0&1 had a problem starting.
Driver Name: usb.inf

I downloaded and installed SP66793 in the hope that this would fix the problem, but no joy. Any assistance is appreciated.

The system is factory.

I'm going to try replacing the front USB/audio assembly and see what happens.

Many thanks for your help!

[EDIT 08/27/2018: Replacing the entire front audio/USB assembly module fixed the problem. It was hardware.]

from device manager, select one with the yellow "!" and then select "uninstall device" it should disappear from the list

now reboot, and windows should find the devices again and possibly fix the conflict that is preventing the ports from working

short of that, a os reinstall should also fix the problem

no 3rd party drivers are required or needed under windows 10 it has the nessary usb 3.0 drivers  built into the OS

last, SP66793 is not a usb driver it is a driver for the HARD DRIVE (AHCI) for windows 7 and lower,  and may cause issues if installed under windows 10

All of the above has been completed several times over with no success. The issue repeats as soon as any device is plugged into the top 2 ports.

Reinstalling 10 isn't an option at this time.

Thanks anyway for your time!

only other thing i can suguest is to try booting from a windows 10 based utility disk/USB key, and see if the util can activate the 2 defective ports. if it can you will know it's a software issue. if the disk has the same issue then a bad USB hub in the chipset may be the issue.

does this system have any cards installed or is it in factory condition?

try bob obhm's util disk (a google link is in the web page listed below)
