Operating System: Microsoft Windows 10 (64-bit)

How and where do you submit the information to get the rebate for the purchase price of my Risk-Free Care Pack that I purchased from HP. I have  filled out the form and do not know where to send it.  There is no address either on snail mail or e-mail.  I have also spent much time  on the phone calling **bleep** 385 5410 and there is never an answer.  Help please.
