How do you do a factory reset as I have lost user name and password

Where is the printer requesting you user name and password? Depends if is in the printer's touch panel or the web interface.

A restore to factory defaults doesn't help if the touch panel is locked by user name and password an EEROM reset from the diagnostics has to be done but if the user name and password is been requested when accessing the web interface what is needed is control access restore from the service menu(3174 pin code) of the printer under the utilities(5494 pin code).
