Below is the failure report generated by the code you posted, minus your PC's serial number.

You will need to remove and replace the defective memory module in your PC.
Since the report gives no Dimm ID, if you have more than one memory module installed in your PC, remove all but one and test each one individually one at a time until you find the one that is producing the code..
Below is the link to the service manual, where you can find the memory removal and replacement procedure.
