Lockup/freeze/long drive failure

Microsoft Windows 10 (64-bit)

It all started with my desktop screen locking up/freezing and unable to get it to function unless I power down and restart which after running a while will randomly do it again. I ran a diagnostic test and it revealed a long drive failure giving a code:Q4ND5S-8SK9TA-GXPQRF-61B913. Please help, I dont understand what this mean

It mean a Hard Drive failure, therefore you need Backups and Recovery Media and then a replacement HDD.

Make, Model and OS please.
SKU: if possible
System SKU: N6K07EA#ABU

Please do not post Serial Numbers.
