HP Z27 27-inch 4K UHD Display

Basically 1 by 1 inputs stopped working. Tried factory reset. Should I submit a warranty request?

Not good, time to see how good the warranty is.

Potentially you might have a problem with the device you are connecting.  If there is a missing ground, ground loop, or improper static shielding, the device may be destroying every port on the monitor you plug it into.

I ran across this problem long ago with external SCSI enclosures.  I shorted out two 1394 connectors plugging a SCSI cable into the case.  It was static electricity and I actually felt the spark.  I replaced the cables with ones that insulated the connector and also sprayed the work area with anti-static guard.
