I have the above PC with an ATI Radeon 3000 graphics card that has two video outputs; one vga and one dvi. Is there a graphics card replacement for this pc that has three video out ports; vga, dvi and hdmi?

The Radeon HD 6450 low profile card might work...maybe you can find a used but tested and working one on eBay.

Thanks. I can't tell from the pictures whether it has a dvi, vga AND hdmi port. Do you know? Also, would I simply remove the card that's currently in the pc and replace with this one?

Well, I was excited until I opened the pc and realized I do not have a slot for a graphics card in this pc. It appears the graphics are built into the pc. The only thing that looks like slots are to short black things in line with each other. At least that's the only thing that I could see where you might insert something.
All I'm trying to do is add an hdmi video out port to this s5704y hp pavilion slimline pc, but I'm thinking that may not be possible. Any ideas?

My goodness, you're right!
The motherboard shows where such a slot would go, but it is not there.
Just the solder points are there.
Right above where those two black PCIe x1 slots are.

They do make PCIe x1 graphics cards but they are very expensive, and not worth installing in a PC as old as yours.
Nor do any of the ones I do see would work in your PC because they require a UEFI BIOS.
So, I don't think you will be able to do what you want to do.

If you thought that half height card would work to add the hdmi, I'm not too sure that's too much money. I'm trying to mirror what's on the monitor on a projector and two samsung smart tvs. I did get it all when using an hdmi to usb adapter, but the pc would only let me extend the desktop on that tv, not duplicate it.

Unfortunately, I don't.
The only PCIe x1 low profile graphics cards, I can find that have 3 outputs are the ones with the nvidia GT710 GPU, and those require a UEFI BIOS.
You may want to consider upgrading your PC to one that has more expandability options--even a refurbished one.
