What is the size of the fan on the CPU heat sink? 30MM... 40MM??

Without the serial number it is not possible to get the exact bill of materials (spare parts)

Fans are measured from edge to edge, NOT from the hole spacing.

The following might be your motherboard but not positive without more info

If you do not have a fan then just measure the width of the heat sink and buy the same size fan. 

You may need to get a 4 pin molex to fan conversion adapter if the new fan has a different connector.

I was unable to find the exact fan, sorry.
