Hello i would like to upgrade my graphics card for gaming curently have nVidia GeForce GTX 745 And Intel core i5 4670 3.40Ghz any ideas?

According to the specs, your HP Pavilion Desktop - 500-425nc is fitted with the Memphis-S motherboard (SSID: 2AF7), and just like I commented here, is fully 24-pin ATX power enabled/functional:
This is highly relevant, since your stock 180-watt ATX power supply doesn't have enough wattage to power a meaningful graphics card. Point is, you can swap your current power supply with any industry standard 24-pin ATX power supply, as long as you watch its dimensions (LxWxH) for fit inside your case.
Look, it all depends on your budget. Personally, I would recommend upgrading your power supply to at least a 400-watt power supply and go for a decent graphics card like a GTX 1650 at a minimum, or consider an RTX 3050, GTX 1660 Ti or GTX 1660 Super -whatever you can afford.
As far as a processor upgrade is concerned, go for a 4-Cores, 8-Threads CPU, such as an i7-4770, i7-4770K, or an i7-4790 -all 84-watt TDP processors, or an i7-4770S or i7-4790S, which are 65-watt processors, and thus run cooler.
