How do I use the HP 3 in 1 media card reader? Do all sizes fit? Do I need an adapter?

No, from its specs
3-in-1 memory card reader
Supports the following cards:
SD
SDHC
SDXC
It actually supports ONLY one physical size because all 3 SD types above have same physical size, only different in specs, speed and storage capacity.
You ONLY need adapter when you use microSD cards which you can find on many phones, tablets.For example
Note: all normal SD, SDHC and SDXC cards have SAME physical size of the adapter above.
You can use the reader like all normal SD card readers (using push/push to insert and remove).
