My monitor stopped receiving a signal from my desktop.  I preformed all the troubleshooting steps including trying a different monitor, but short of opening up the device and physically messing with the cards.  The device is under the HP 2 years 3 day Onsite HW Support with Accidental Damage Protection for Consumer Desktops warranty.  I contacted the nearest listed service center, and they advised me to fill out a warranty claim before coming in (it's an hour drive to get there).  However, I can't seem to find a way to fill out the claim to be sure I won't be charged for the repairs.  How do I file a claim?

and they didn't tell you how to do it, interesting..
