I have an issue with a HP 4345 which has a 41.3 error from all trays (MP,2,3,4,and 5). I took out the power supply and cleaned all sensors under the registration assy, they seem to work ok in diagnostic mode.
The printer will printer one sheet perfectly fine, then up comes the 41.3 incorrect paper size detected message.
I've checked all the trays, the size detect levers on all trays and they are all ok and intact.
Could someone possibly advise on what to check next?
As I said at the beginning, it's happening from all trays.

A 41.3 unexpected size error will occur if printing custom media sizes or 12x18 media. Banner printing is included in the custom media sizes. If printing 12x18 media from tray 3 through 5 you will see this error. All PDL’s are affected.
Solution:
A fix will be coming in the next Firmware roll that is scheduled for Late Spring 2009. A Firmware version greater than 04.044.0 is what you will be looking for.
Workaround:
If printing 12x18 size media from tray 3 through 5, try choosing to print from tray 1.
Custom media sizes do not currently have a workaround.

Have you removed tray 1 and cleaned checked the pre-feed sensor for its operation? When you completely remove tray 1 use compressed air to clean inside.
Maybe the return spring is giving you trouble, a very small amount of oil on the contact point of sensor lever and shaft will make it run smoothly.

Hello,
To start you could test with another size media to rule out the media.
If you are seeing the issue from Tray1(Manual Feed tray) as well, then I would suggest to look at the common paper path for all the trays. The registraion assembly which detects the media length and width. The registration assembly which holds the width sensors and the registration sensor(TOP). If the rollers in the assembly are worn out then I would surely suspect the registration assembly causing the timing to slip.
To confirm the part you could set tray1 to any size/ Any type and test if the error shows up.

All sensors test fine in the Diagnostic sensor test.

Remove tray 1 assy clean and check the pre-feed sensor for its operation. Use compressed air to clean inside sensor, check return spring, use half drop of oil to make sensor lever run even more smoothly.
It is a 10-minute job.
Forget about registration assembly, problem sensor that you need to check is not on registration assy it is on prefeed assy.

I dont think the pre feed sensor comes into play when printing from tray1(manual feed) unless you are printing duplex jobs from tray1. Does it show the error from Tray1 when size/type is set to any size /any type? There could be a lag in the TOP sensor flag returning to the home position. Also you could try printing the straight path and test. The media will not get into the reversing assembly. Copy->Output Bin Paper Path->Face up (Straight Path)

With ref to 41.3 there are a list of errors which cannot be specific to one particular reason
I have a list of errors related to 41.3
41.3 LOAD TRAY XX <TYPE> <SIZE>
Description
This is typically caused if two or more sheets of media stick together in the MFP or if the tray is not adjusted correctly.
Recommended action
Reload the tray with the correct media size. Reconfigure the size in a tray so that the MFP will use a tray that contains the size required for the print job. Follow the instructions in the onscreen dialog box.
41.3 LOAD TRAY XX <TYPE> <SIZE> - To Use Another Tray Touch OK
Description
The specified tray is loaded with media that is longer or shorter in the feed direction than the size adjusted for the tray.
Recommended action
Touch OK to use another tray. Reconfigure the size in a tray so that the MFP will use a tray that is loaded with the size of media required for the print job. If the message does not clear automatically from the control-panel display, power off and then power on the MFP.
41.3 MEDIA TOO SHORT CANNOT DUPLEX
Description
The MFP cannot print on both sides of the selected media because the media does not meet the minimum length specification.
Recommended action
Select single-sided printing or load a longer-size media. Resend the job.
41.3 MEDIA TOO SHORT CANNOT SEND TO FACE DOWN OUTPUT BIN
Description
The MFP cannot deliver the selected media face-down because the media does not meet the minimum length specification.
Recommended action
Select the face-up output option, or load a longer-size media in the tray. Resend the job
All these are related to pick up issue
My recommendation is to first check the Maintenance kit Percentage
I suspect that all the rollers with respect to the trays might be worn out
Please try checking the page count and Maintenance kit remaining percentage
and if its low and have to replace it get this part ordered through HP
