Operating System: Mac OS X 10.6 Snow Leopard and older

I am trying to print mail merge envelopes and the file looks perfect on my computer.  When I send to print (envelopes are loaded correctly in the tray) all I get is a completely black filled rectangle.  I've tried printing directly to printer, saving as a PDF and sending to print, and anything else I can think of.  Needless to say, it's using a lot of ink! Any ideas?
