My HP Officejet 4650 printer works fine for everything it is designed to do with one problem. When I go to print Avery Label # 5215 a 7-label sheet from the downloaded template as Pdf or Word the print in print preview look fine click print and it prints the first label at the very top of the sheet spacing the rest equally distance Missing all the labels. How do I fix this?

This is a smaller label sheet - 4x6 inch sheets.
The printer does not specifically support this size label sheet.
Reference
The printer does support "Card" paper in this size.
and
The printer supports Brochure paper, which is the "right thickness".
but...
Brochure paper does not come in 4x6 inch sheets.
You'll need to see if you can adjust the paper type and get it to also accept the size.
What to do?
The following solution assumes (requires that) the Full Feature Software / full driver software is installed.
Start...
You can perhaps print the label sheets.
First,
Alert the printer to the paper size / type loaded in the printer.
Example shows how to check / adjust at the printer display AND how to use EWS to adjust Tray and Paper Management.

Next,
Create your completed label - perhaps in Word.
Next,
Export the completed Word document / template to PDF.
Open the PDF file in Adobe Reader DC
Click the Print icon
Set the options in the main menu
Open Properties
Set the paper size and paper type as can be done using what you find in the various size / type lists...
For example, Set the Shortcut to Everyday Printing > adjust the settings in the other Tabs to change the paper and look for an acceptable "paper type". For example, look for Media / type : Card, Thick, Brochure
What if? It is possible that the labels can be printed using the paper type "Plain paper" - this is not a "sure thing" since labels are generally a little thicker than plain paper. Worth trying if other options balk.
Click OK to save the changes in Properties and exit that menu
Back on the main print menu, Click Print

What if?
It might be possible to create a Custom Paper Size form if an acceptable combination of size and type cannot be found.
This is a bit more complicated (extra step to create the custom form).
Then, the software has to accept the new form and add it so you can use it...

Reinstall / Update your PDF Reader (Adobe Reader DC /Optional Foxit Reader)

Only one very small comment I wanted to the excellent info that @Dragon-Fur already provided you:
I print labels all the time, nearly every day, and the app I use has an option to print an alignment sheet -- with vertical and horizontal lines. Then, I hold the page against the labels sheet, and in the same app, make the necessary alignment changes to ensure the printout is centered on each label.
I don't use Word or other apps, so you need to check if the app you use can print an alignment sheet.
