Simply inserting an SD card adds extra storage to your device. If you can see D:\ in windows explorer, then it is ready to go.
The steps I outined earlier is a method of fooling the OS into thinking that the removeable drive D:\ is an extension of C:\ and allows some programs that insist on using fixed drives only to be installed. It will NOT move existing installations over to the SD card. To do that you will need to do the following:
Manually add the folders "Program Files" and "Program files (x86)" to D:\
Make sure when you use Windows explorer to look at C:\ the folder you used to extend the SD card appears with a different icon:
Uninstall any programs you want to move and re-install them , being careful to choose the option of which folder to intall to, and choose either D:\Program Files directly, or C:\SDCard\Program Files if it refuses to use it.
If you use Dropbox, OneDrive or any other cloud storage, they also consume space on C:\. Each one has a different method of moving the location and most will only use the C:\SDCard route to do so.
You can also create a new Documents folder on D:\ and move the location of MyDocuments. Look up the method for doing that.
