I have the same issue on my HP Victus 15-fb0xxx with the MediaTek MT7921.

I updated everything to the latest HP versions (BIOS, Bluetooth driver 1.3.17.168, and Wi-Fi driver 3.0.1.1337), but the problem still comes back. Bluetooth works for a while, then the MediaTek Bluetooth Adapter shows Code 43, and sometimes it later changes to Unknown USB Device (VID_0000 & PID_0002). An EC reset temporarily restores it, but the issue eventually returns.

Have you found any permanent solution? If HP Support replies to you, please let me know. I'll do the same if I find a fix.

The MediaTek MT7921 dropping into Code 43 and changing to an Unknown USB Device is a known hardware-level firmware crash. The most reliable permanent fix for the MediaTek MT7921 is to replace it with the Intel AX210 (non-vPro version). It is a cheap, drop-in replacement that takes 10 minutes to install and permanently solves Bluetooth and Wi-Fi stability.
 

To attempt one last software fix before replacing the card, try modifying the USB power management: 
 

  1. Open Device Manager, click View, and select Show hidden devices.
     
  2. Expand Universal Serial Bus controllers, right-click on the Unknown USB Device (or MediaTek USB port), and select Uninstall device
     
  3. Expand Bluetooth, right-click MediaTek Bluetooth Adapter, and select Uninstall device (check the box to attempt to remove the driver software if prompted).
     
  4. Right-click the top name of your computer in Device Manager and select Scan for hardware changes.
     
  5. Next, expand Network adapters, right-click MediaTek MT7921 Wi-Fi Adapter, and go to Properties.
     
  6. Switch to the Power Management tab and uncheck the box that says Allow the computer to turn off this device to save power.
     
  7. Shut down your PC completely, unplug the charger, hold the power button down for 45 seconds (hard reset), plug the charger back in, and turn it on.
