Hello I have a Pavilion dv6-6145dx and I have tried installing drivers for the USB ports 2 are working fine the other two are not plug and play does not recognize and sends up an error code "The drivers for this device are not installed. (Code 28)" I have installed USB 3.0 Host Controller Utillity for this model and only have two USB ports working the ones I don't have working have a Hardware ID's of PCI\VEN_1022&DEV_7812&SUBSYS_358B103C&REV_3 not sure why. I have tried to uninstall the divice to see if plug and play picks it up but no luck. I have tried to go and manually give the file path of the driver to the 7z file. I have even tried to give it the same file path as the other USB ports from its drivers to see if it would sense the drivers no luck. I am at a loss.. any help would be greatly appreciated

Those are the drivers you need.
Here is a partial copy and paste of the USB3 driver setup information file which shows driver support for the hardware ID you posted.
[SourceDisksFiles.amd64]
amdxhc.sys = 1, \x64
[DestinationDirs]
DefaultDestDir = 12
[Manufacturer]
[AMD.NTamd64.5.1.1]
%AMDXHC.DeviceDesc% = AMDXHC, PCI\VEN_1022&DEV_7812
%AMDXHC.DeviceDesc% = AMDXHC, PCI\VEN_1022&DEV_7814
I have two suggestions for you to try and if they don't work, I suggest you reinstall W7 and install these drivers now that you know which ones you need. The usb filter driver gets installed after you install the usb 3 driver.
OK...first suggestion:
Try manually installing the driver as follows...
Go to the device manager and click on the usb controller needing the driver.
Click on the driver tab. Click on update driver. Select the Browse my computer for driver software option and browse to the driver folder that was created when you ran the file.
That folder will be located in C:\SWSetup\sp61462.
Make sure the Include Subfolders box is checked and see if the driver installs that way.
Then reboot.
If that doesn't work...
Second suggestion: Install the chipset driver package directly from AMD, and reboot. 1st file on the list.

Ok tried the first suggestion got:
Windows found driver software for your device but encountered an error while attempting to install it.
AMD USB 3.0 Host Controller
the system cannot find the file specified.
I you know the manufacturer of your device, you can visit its website and check the support section for driver software.

See if this works...Do it with the first USB driver I posted (sp61462)
Go to the device manager and click on the usb controller needing the driver.
Click on the driver tab. Click on update driver.
Select the Browse my computer for driver software option, and browse to the driver folder that was created when you ran the sp61462.exe file.
That folder will be located in C:\SWSetup\sp61462.
Make sure the Include Subfolders box is checked and see if the driver installs that way.
