Mac OS X 10.10 Yosemite

I am trying to print on a 3x5 index card. Everything is formatted correctly on my computer and in the printer. The printer picks the paper up just fine but then prints out a blacked out page. It isn't inverted coloring, it's just ALL black. I have changed to print on other paper for other documents and it prints just fine. What is going on?

I understand that you are facing issues with your HP ENVY 4511 All-in-One Printer wherein the 3x5 index card is printed all black.

 

This issue could occur if the drivers are not configured properly.

No it still didn't fix the problem.

This did fix the problem I was having with getting my printer and computer communicating properly though. So, for that I thank you. But it is still printing out a completely black page for the index card.

I reviewed your post and it looks like you've pretty much done, everything one could do to resolve such issues,

If you've gone through the article: Fixing Print Quality Problems already (the entire length of steps) and yet the issue persists,

The best thing to do is to Contact HP phone support for further assistance:

 

Please use the following link to create yourself a case number, then call and it may help speed up the call process:

Step 1. Open link: www.hp.com/contacthp/

Step 2. Enter Product number or select to auto detect

Step 3. Scroll down to "Still need help? Complete the form to select your contact options"

Step 4. Scroll down and click: HP contact options - click on Get phone number to obtain a case along with the phone number.

They will be happy to assist you immediately.
