The troubleshooting steps in my post here may help resolve custom page sizes in Windows 7 for your HP Envy 5540 printer.

The steps should be the same for Windows 7 and regardless of the printer. Go to the Devices and Printers folder and single left click on your printer. Now look at the top of the Devices and Printers, the Print Server Properties will not appear until you select the printer. Once you have clicked on the printer the Print Servers Properties will appear. Click on this and go through the steps I outlined to make a new form.

The list you give would result from RIGHT-clicking on the printer (or LEFT-clicking if the buttons on the mouse were reversed). Click the opposite button on the mouse on the printer and it will list the new commands in the upper line of the Devices and Printers folder.
