Mac OS X 10.11 El Capitan

Lately when I try to print anything, there are lines missed, the font is really small and weird and some lines it only prints either the top  or bottom half of the words.

Anyone else have this issue with El Capitan? I'm not even sure it's a problem with El Capitan, as I have the same issue when I print from my iPhone 6.

 

I've tried cleaning the print cartridges, aligning the paper, etc. I did all the diagnostic stuff and here's what prints out...

This sounds like an electrical connection issue between the cartridge and printer. The troubleshooting steps in the document here may help resolve partial lines printing on your Envy 4500.  I would suggest cleaning the cartridge and printer's electrical contacts as shown in step three.

Nope. Tried those steps and this was the result... Looks even worse.

For a print quality issue like the one you are seeing, where the black swath is not consistent across the page, the troubleshooting steps in the document here may help.  You may need to run the cleaning cycle in step 10 up to three times.

 

If the steps in that document (short of replacing the black cartridge) do not resolve the issue then you might try the following:  get a coffee cup and put about 1" of hot but not boiling water in it.  Put the black cartridge in the teacup nozzles down and let the cartridge soak in the hot water for ten minutes.  Remove the cartridge and gently blot the printhead on a damp tissue.  Dry the electrical contacts with a lint free wipe - a clean coffee filter works well for this.  Put the printhead back in the printer and try printing a test page.
