We are getting frequent paper jams on this printer.
I recently installed a maintenance kit, but no improvement.
Printer has printed 202,255 pages, and has had 16 jams in the last 3,218 pages printed.
Isn't this excessive?
Any helpful input would be greatly appreciated!

If you installed the MK I assume you replaced both the pickup roller and separation roller for tray 2. If you did not, then that is the first thing to do. You will want to check out the backstop of tray 2. Make sure the little arrow is exactly on the letter setting and not a bit forward as that will impede the movement of the paper up when the tray lifts. Also always check the front part of the ream of paper that sit in the front of the tray to make sure it is smooth and I would fan it. You may want to maybe try another paper vendor just to make sure the paper is not causing issues. To be honest 16 jams over 3000 pages which is 6 reams of paper is not that excessive but is more than normal.

Another likely cause of jamming is the feed rollers behind the manual feed tray. If you remove that unit theres a set of EPDM rollers behind (Youtube is your friend for this).
These rollers become smooth and cause the paper to slip occasionally or fail to feed all together. If you get platen cleaner and a microfibre cloth these can be cleaned up.
As the other poster mentinoned if you havent already changed the feed and seperation rollers.
The fuser unit can also cause their fair share of jams, but if youve fitted a MK this shouldnt be a problem as you will have changed it when doing the kit.

Your getting alot of paper late jams which means its not where it should be when the printer is looking for it. This does suggest the paper is slipping at some point of getting caught.
13.01.00 normally refers to a jam in tray 2. So id definatly be looking in the above mentioned areas.
