This issue has been raised before and has been around for a long time. It should have been solved by HP ages ago instead of their staff offering workarounds. They should make the latest driver unavailable until they have a fix for it. I'm not actually asking a question but giving an easy workaround that worked for me. I managed to get the colour / black & white radio buttons back in the print window by picking the driver named 'HP ENVY 4500 Series Class Driver' rather than the faulty driver 'HP ENVY 4520 Series' from the list in the Add Print Driver Wizard. You can get to the Wizard by selecting Printer Properties in Settings, then clicking on the Advanced tab, then New Driver button.
