On the HP Touchsmart 6100, what is the USB drive plugged in horizontally at the bottom of the back of the monitor?
It looks like a switch of some sort until you push or pull it enough to pop it out - see attached photos of the same item at various angles.
It didn't seem to mount in the side usb ports but the computer makes the expected noises when it's inserted and removed - plus the small circle lights up with a blue light for about 2 seconds and then turns off.
Do I need to leave it in the machine?

Firstly I have no idea what is HP Touchsmart 6100. I know HP has many machines called HP Touchsmart 610-something Did it come with keyboard and mouse ? Most likely it is the receiver for KB/MOUSE set.

Indeed, it is undoubtedly the keyboard / mouse receiver
so if you have the original keyboard / mouse kit, you have to leave it yes!
- Make sure that the USB receiver is plugged into the port for the wireless receiver.Figure : Location of the receiver plugged into the port
