hp omen 15-ax250wm charger jack gets extremely hot

recently i noticed whenever i plug in my charger the jack gets extremely hot, so hot its burn me countless times and hot enough to melt the plastic round the chargin port, i opened it up and notice my battery is swollen, could the swollen battery be the course or another effect caused by the hot charger. the jack doesnt get hot when unplugged, when laptop is also off.
