Printer driver shows printer is online and working
The out of paper light is blinking and the low toner light is lit. I know the toner is low, but it's not out of toner yet. The paper tray has plenty of paper in it but the light still blinks and the printer will not print anything. The job eventually times out.
I have performed a cold reset.

When you restart does the printer appear to come to ready? Are you able to print the demo page from the printer control panel? If you can then it is possible that either the printer or the driver has a wrong paper type setting and the printer is waiting for you to load the correct paper. What happens if you put paper in the MP tray when the light is flashing and then press the go button?

It almost sounds like a fatal error, typically when the formatter needs to be replaced. Try performing an engine test to see if the basic print engine can print:
Simplex engine test page
1. Open and close the print-cartridge door three times within 2 seconds.
2. The engine-test page prints with a test pattern on one side of the paper.
If the printer can print the engine test page, but the lights are still the same, then you need to replace the formatter. If it doesn't print, then you may have a hardware problem somewhere inside. Check and clean the pickup/feed rollers, check the paper path, make sure the paper tray paper size guides are set correctly and not too tight against the paper as that will hinder the paper from lifting correctly.
