I have a Elitebook 860 G9 laptop that I use closed with an external screen + Bluetooth keyboard/mouse. When the laptop is closed the air vent outlets is literally blocked. The only way for hot air to leave the laptop is this 2mm small gap shown in the picture below. The air vents is barely visible through the gap.
This seems like e really dumb design and I'm worried if it is even recommended to use the laptop this way, not risking it to overheat?

It will be fine. I have a Zbook G3 I am using right now with the lid closed, keyboard, mouse, dual 4k monitors connected with a Zbook Thunderbolt dock. Plenty of hot air gets out through that gap, which on mine looks very similar to yours. I have monitored temperatures and it runs no hotter with the lid closed than running as a standard laptop with the lid open. Just keep the area around that gap clear.
